The Wasilla Senior Center now has Care Coordination services to help Medicaid Waiver clients and potential clients with:
● Information
● Assessment and Evaluation of client's needs
● Plan of care
● Coordination with care providers
● Ongoing care monitoring
Waiver clients are individually planned and may include:
● Respite care
● Environmental Modification
● Transportation
● Adult day care
● Regular Medical equipment services, such as prescriptions, office visits, and personal care attendants
● Chore services
● Specialized private duty nurses
● Care in assistant living
● Home delivered meals
